I don't really see why a template is needed, every ban appeal will be different cause people are banned cause of different reasons.

Server banned from:
Rank before ban:
Reasons why you were banned:
Reasons why we should unban you?:

blah blah.

If a player doesn't realise he has to mention those things in his post then I don't think that he should be unbanned either, if we need more info, then we can still ask for it.
